The CORE program is dedicated to providing a wide range of social services, wellness linkage, and care coordination support to all members of the Monterey County community.

Who is eligible?

Individuals must be 18 or older and reside in Monterey County.

Self-Referral:

Are you a provider or community agency looking to refer someone to our program? Please use our secure form to send a referral for the person you're helping. Submit Provider/Agency Referral Form

Below are the services our program offers. Click on each item to learn more!

 

  • * Support in accessing essential community resources that address Social Determinants of Health

    *Informing individuals about available employment and training resources, empowering them to explore and pursue new opportunities

    *Assist in completing applications and support navigation in order to enroll in community resources

    *Examples: Support in applying for housing, treatment options, finding food, job fairs and employment information, and more!

  • * We provide hands-on assistance with applications for Medi-Cal and other health insurance programs

    * Helping find a Primary Care Provider (PCP) and scheduling doctor appointments to ensure timely and personalized healthcare.

    *Gain insights into various health insurance options with our guidance on obtaining the necessary documents for well-informed decision-making

    *Examples: Assistance with Medi-Cal applications, finding a Primary Care Provider (PCP),  other health related services, assisting with transportation to appointments, and more!

  • * Our dedicated case management support ensures personalized assistance, making certain that individuals not only get connected to relevant programs but actively participate in them

    * Accommodate to meet individual needs, recognizing the uniqueness of each person's situation and providing personalized assistance for the best outcomes

    *Participate in educational programs and workshops covering essential topics such as preventative care, substance abuse awareness, and mental health

    *Access to Naloxone (Narcan) training and education to empower individuals with life-saving knowledge.

Free Medi-Cal Application Assistance Event | Asistencia Gratuita para la Solicitudes de Medi-Cal

Introduction: Do you need help completing the Medi-Cal form? You could be eligible for free or low-cost health services if considered eligible, even if you do not have a social security number. To find out if you qualify join us and we will help you complete your application and answer any questions!
