ADRC Searchable Resource Database - Under Construction
The MCADRC is a partnership between the County of Monterey Area Agency on Aging, the Aging and Adult Services Branch of the Department of Social Services, and the Central Coast Center for Independent Living. The MCADRC, and its extended partners, work together to better serve the community by providing a “No Wrong Door” approach to information, referral, and assistance. Our goal is to provide no-cost, objective information about the full range of options available to streamline access to services and supports in the community. Services are provided to individuals regardless of age, disability or income.
Services include:
- Enhanced information, referral and assistance – comprehensive resource information, follow-ups, and referrals
- Options Counseling – assistance with making informed decisions about long-term services and supports
- Short-term Service Coordination – expedited access to services and supports, for up to 90 days or until a longer term plan is in place
- Transition Services – support while transitioning from hospital to home or skilled nursing center to home to ensure success in the least restrictive environment
