Telescope Lending Agreement

As a holder of a Monterey County Free Libraries (MCFL) card and the person checking out a telescope, I agree to take sole responsibility for the telescope, and all parts and accessories, and will adhere to the following guidelines:
- The telescope may be checked out by a member of MCFL who is 18 years or older and in good standing. (Patron must present a valid California Driver’s License or government issued picture I.D. along with their library card at time of check out.)
- The telescope must be returned directly to a staff member at the Help Desk at the Seaside Branch Library. Please do not return the telescope in the book drop or leave it in the library unattended.
- When the telescope is being transported in a vehicle, it must be secured with a seatbelt.
- The loan period is 3 weeks, with one (1) possible renewal if nobody has a hold on the item.
- The overdue fines are 25 cents per day. If the telescope is not returned within two weeks (14 days) of its due date, the full replacement costs below will be charged to the responsible party.
- I accept full responsibility for the telescope and accessories while checked out to me, including:
- I am liable for repair costs if the telescope, or any part or accessory, is returned damaged.
- I am liable for replacement costs (outlined below) if the telescope, or any part or accessory, is lost or stolen while checked out to me.
- Any costs incurred will be added to my library record, will constitute a debt owed to the MCFL and must be paid in full.
- I am responsible for and will follow all rules and guidelines for safe use of the telescope while it is checked out to me and will make sure that all users follow the rules and guidelines.

Children under age 18 must be supervised by an adult while using the telescope.
CAUTION: DO NOT LOOK AT THE SUN WHILE USING THE TELESCOPE; DOING THIS CAN BLIND USER. THE LIBRARY IS NOT RESPONSIBLE FOR ANY DAMAGES CAUSED BY THE USE OF THE TELESCOPE.
